Not for Public Use. The form can be found on pages 16 to 17 (English) and 23 to 24 (French) of the Adult Abuse Registry Regulation https://web2.gov.mb.ca/laws/regs/current/_pdf-regs.php?reg=164/2012. The Adult Abuse Registry is a database which records the identity of individuals who have been found to have abused or neglected vulnerable adults. This is the form that notifies the person that their name will be placed on the registry
