This document is a medical mileage transportation form that provides specific details of medical appointments, and transportation costs to be completed by the EIA participant and EIA staff.  Receipts for bus fare and parking costs must be attached when submitting this form to be processed.

A signed 3rd Party Contractor Declaration Form is submitted to the Province each time  a tax credit claim request made to the Interactive Digital Media Tax Credit program by a participating corporation includes one or more 3rd Party Contractor Invoice(s).  This form is used to verify business information of the 3rd Party Contractor and provides consent to have that information gathered and shared for program purposes pursuant to the parameters of Canada's Freedom of Information and Protection of Privacy Act (FIPPA).

Corporations participating in the Interactive Digital Media Tax Credit (those that have previously received a Certificate of Eligibility for a proposed project) will use this form to accompany their annual request for a tax credit certificate.   This form provides current corporate information and financial actuals pertaining to project expenditures in support of a requested tax credit amount.
The Manitoba Interactive Digital Media Tax Credit (IDMTC) is a refundable corporate income tax credit that provides up to a 40% credit on qualified labour expenditures, and some marketing expenses, incurred in the development of an eligible interactive digital media product that is brought to market.  To be eligible for credit, a corporation must first apply for a Certificate of Eligibility from the Province before proposed project work on a product begins.
A signed copy of this Program Declaration and Consent Form accompanies all corporate applications for participation in the Interactive Digital Media Tax Credit program.  A signed Corporate Declaration is also submitted each time a tax credit claim request is made.  Program Applicants use this form to attest to the validity of the documents they are providing, and to consent to having information gathered and shared for program purposes within the parameters of Canada's Freedom of Information and Protection of Privacy Act (FIPPA).

A signed Employee Declaration and Consent Form is submitted to the Province for each employee who works on a qualifying project for the Interactive Digital Media Tax Credit program each time a tax credit claim request is made by a participating corporation.  This form is used to verify employee information and provides the employee's consent to have that information gathered and shared for program purposes pursuant to the parameters of Canada's Freedom of Information and Protection of Privacy Act (FIPPA).

These guidelines provide general information on Manitoba's Interactive Digital Media Program (MIDMTC), including its eligibility criteria, instructions on applying to have a planned project entered in the program (prior to project commencement), and how to make a claim for labour expenses undertaken on approved projects.

This is a Funding Request form to be used by applicants seeking funding for an eligible specialty program or course under the NRRF Specialty Programs and Courses Funding policy. Applicants are required to provide detailed program or course information, a description of projected benefits, and details regarding projected expenses. The form is filled out by Manitoba nurses, and is processed by the Nurses Recruitment and Retention Fund Committee.

Clean Environment Commission Hearing Participant Assistance Application Form.  The Participant assistance program provides funding or other assistance to qualifying people and organizations to ensure effective public participation in the environmental assessment and review of major projects. Any person or group of people who intend to participate in a public hearing may apply.

This Annual Report and Information Return must be filled out by a corporation after its first year of participation in the Small Business Venture Capital Tax Credit Program.  A corporation must complete and submit one copy of this return, including all attachments, together with a copy of professionally reviewed financial statements encompassing the reporting period indicated.

A Short Form Annual Report and Information Return must be provided by a corporation following both its 2nd and 3rd year of participation in the Small Business Venture Capital Tax Credit Program.  A corporation must complete and submit one copy of this return, including all attachments, together with a copy of professionally reviewed financial statements encompassing the reporting period indicated.

List of on-going responsibilities for corporations participating in the SVBCTC Program pertaining to share issuance, share ownership and reporting.  An Authorized Signing Officer of the participating corporation must sign and return this form to acknowledge receipt and acceptance of the list of responsibilities.

The Small Business Venture Capital Tax Credit (SBVCTC) provides a non-refundable Manitoba tax credit of up to 45% to individuals and corporations who acquire equity capital in eligible Manitoba enterprises, with a minimum investment of $10,000 per investor.  In advance of issuing shares for planned investments, a Manitoba corporation must submit this Application Form to gain pre-approval for the corporation to participate in the program which allows them to issue shares eligible for a tax credit for qualifying new investments.

Individuals or corporations that invest in a business participating in the Small Business Venture Capital Tax Credit Program must complete and sign a copy of this form to affirm the eligibility of their investment for a tax credit and to acknowledge the risk involved in making their investment. This form also provides permission for information provided by the investor to be used for program purposes pursuant to the rules and standards of Canada's Freedom of Information and Protection of Privacy Act (FIPPA).

Corporations participating in the Small Business Venture Capital Tax Credit Program must provide each eligible investor with a receipt for their eligible share transaction for tax credit purposes.  Participating corporations use this form to generate tax receipts for eligible investments and must be signed by an Authorized Officer of the share issuing corporation.

The tax release form grants permission from an EIA applicant/participant for Revenue Canada to release Income Tax information to the Department of Families for the purpose of information verification.

This is an application form requesting an EIA Intake Appointment.  Staff at Welcome Place (Settlement Services) help the applicant to complete this form and then send it to EIA Programs on their behalf.  An appointment date will be set and then sent back to the staff at Welcome Place (Settlement Services) who will share this information with the applicant.
