Corporations participating in the Community Enterprise Development Tax Credit Program (CEDTC) must submit an Annual Subscription Proceeds Report.  Elements include documenting the use and impact of Subscription Proceeds received in the taxation year, the number and type of jobs created, the value of assets acquired and the number of new investors.

Corporations participating in the Community Enterprise Development Tax Credit Program (CEDTC) must complete an Annual Share and Tax Credit Receipt Issuance Report verifying the name, address and Social Insurance Number of eligible investors, as well as the number and class of shares issued, the total amount paid by the Investor and the amount of the Tax Credit issued.

The Community Enterprise Development Tax Credit (CEDTC) provides a refundable Manitoba tax credit of up to 45% to individuals and corporations who acquire equity capital in eligible Manitoba community-based enterprises.  Prior to issuing shares for planned investments, a Manitoba corporation must submit this Application Form to gain pre-approval to participate in the program and verify its status as a community-based enterprise.

Corporations participating in the Community Enterprise Development Tax Credit Program (CEDTC) must provide a letter from their financial institution confirming that they have established a separate account for proceeds of the sale of shares issued under the CEDTC Program.

Individuals that invest in a business participating in the Community Enterprise Development Tax Credit Program must complete and sign a copy of this form to acknowledge the risk involved in making their investment. This form also provides permission for information provided by the investor to be used for program purposes pursuant to the rules and standards of Canada's Freedom of Information and Protection of Privacy Act (FIPPA).

Corporations participating in the Community Enterprise Development Tax Credit Program (CEDTC) must provide each eligible investor with a receipt for their eligible share transaction for tax credit purposes.  Participating corporations use this form to generate tax receipts for eligible investments signed by an Authorized Officer of the share issuing corporation.
This form is used by collectors to report the tax owing on cigars under the Tobacco Tax Act.  This form is completed in conjunction with the Tobacco Tax Return T-3 form and the Tax Remittance Schedule T-7 form.
