MHRC SHRP Payment Plan Agreement
An agreement between a tenant and Manitoba Housing to make monthly payments on their account on a monthly basis over a period of time to bring their account into good standing in lieu of being issued a notice to terminate the tenancy agreement.

MHRC SHRP Unit Condition Report
A Unit Condition Report provides a detailed description of the condition of a rental unit at move in and move out and is signed by both the landlord and the tenant. A unit condition report can protect both the interests of the landlord and tenant by documenting the condition of the unit and determining who is financially responsible for damages and repairs.

MHRC SHRP Unit Offer Letter
Where mail is the only method of communication, a letter is used to inform an applicant that housing that meets their needs has become available in a location they had requested.  They have 10 business days from the date of the letter to contact housing or the offer will be cancelled.

MHRC-RHP Statement of Income Declaration (Vacant)
When applying to the Rural Homeownership Program each household member, 16 years or older, is required to submit this Statement of Income Declaration along with other supporting documents identified in the declaration.

If you disagree with a decision, you can appeal to the Social Services Appeal Board.  Appeals may be made about decisions for these programs/services: adoption agency licensing, child care licensing and subsidies, 55-plus income supplement, prenatal benefit, income assistance, residential care licensing, marketAbilities program eligibility and vulnerable persons living with a mental disability (eligibility and individual care plan). Use this form to make an appeal.

Notice of Application (CFS-11)

Notice of Intended Entry on the Adult Abuse Registry (Form AAR-3)
Not for Public Use.  The form can be found on pages 16 to 17 (English) and 23 to 24 (French) of the Adult Abuse Registry Regulation https://web2.gov.mb.ca/laws/regs/current/_pdf-regs.php?reg=164/2012. The Adult Abuse Registry is a database which records the identity of individuals who have been found to have abused or neglected vulnerable adults. This is the form that notifies the person that their name will be placed on the registry
