Agreement between Minister of Transportation and Government Services, City of Winnipeg and Contractor that sets out conditions and procedures for disclosure and protection of vehicle owner information, in order to generate Offence Notices.

The Amendment Agreement to 2002 Information Disclosure Agreement to Support Photo Enforcement, signed on January 1, 2014 (2014 Amendment Agreement) amends the original 2002 Agreement Respecting Disclosure of Motor Vehicle Registration Information (2002 Agreement) by changing the names of the parties to the Agreement. Because authority for the administration of  driver and vehicle licensing was transferred to Manitoba Public Insurance (MPI) from Manitoba Infrastructure, the 2014 Amendment Agreement makes MPI the responsible party (on behalf of MB) and reflects a name change for the same contractor, as the business changed their name from ACS Public Sector Solutions Inc. to Xerox Business Services.

The Second Amendment Agreement to May 31, 2002 Information Disclosure Agreement to support the Photo Enforcement Program, signed in February 2018 (2018 Second Amendment Agreement) also amends the original 2002 Agreement to reflect the name of the contractor changing again to Conduent Business Services Canada Inc. from Xerox Business Services Canada Inc. on April 11, 2017.

The Confirmation of Attendance Form is a written record used to confirm the attendance of children enrolled with Children's disABILITY Services to appointments for disability-related professional services for the purposes of receiving transportation assistance funding. A section of this form is completed by the service provider and another section is completed by parents or guardians.

This document is the template of a letter sent by Children's disABILITY Services to notify families of the approval of a plan to subsidize some expenses they will incur when bringing their child to professional services that are not available in their community. Each letter will describe the details of a family's approved transportation assistance plan and it includes instructions for families on how to request reimbursement of approved travel expenses.
