This form is used when a CFS agency makes application for an order requiring the parents/guardian of a child in care to pay to the agency maintenance for the child(ren) in accordance with a maintenance agreement executed by the agency and the parents/guardians under subsection 15(2) of The Child and Family Services Act.This is a statutory regulated form embedded in The Child and Family Services Regulation.
