Forms and Policies

The Parental/Legal Guardian Consent for Photographs form is used by the child and family services agency caseworker and the parent/legal guardian of a child. It gives permission for the caseworker to take a current photograph of the child for the child's file and is kept in the Child and Family Services Application.
