This form is used to collect information regarding circumstances of post-exposure to blood and bodily fluids potentially carrying human immunodeficiency virus, where human immunodeficiency virus post-exposure prophylaxis starter kits were provided. This form is applicable to health care professionals at initial contact of care as well as those responsible for providing follow-up care. This form is processed by Manitoba Health, Seniors and Active Living.
