Forms and Policies

This form must be completed and submitted to the department by any external organization that employs an individual responsible for a security breach involving personal health information that is maintained by the department. This form is applicable to any trustee as defined by The Personal Health Information Act.  This form is processed by the branch that is responsible for the personal health information involved in the breach.
