This form is a referral to the Dialectical Behaviour Therapy Program in adherence with set guidelines, which the Selkirk Mental Health Centre intake team reviews. This form is available to external agencies within the Regional Health Authorities of Manitoba. This form is enforced by Manitoba Health, Seniors and Active Living.
