This form is to be completed by physicians providing annual management of diabetes to their patient, including development of a patient care plan.  This form is applicable to physicians submitting claims for the diabetes, chronic disease management tariff.  This form is processed by the Claims Unit, Insured Benefits, and Insurance Division.
