The form is not an externally available form, as the request for meter dispute test is made verbally by customer to Manitoba Hydro. When the request is made, Manitoba Hydro staff initiate the process and fill out the form on behalf of the customer and submit it to the Customer Metering Department, who then submits the request to Measurement Canada.
