Community Living disABILITY Services - Enhanced Program Support Review and Approval Checklist
This form is completed by regional program managers within the Department of Families to support the approval of enhanced program support for Community Living disABILITY Services program participants with high or complex support needs, or participants with low support needs, but who require intensive system and case coordination.
