This form gives permission to Manitoba Health, Seniors and Active Living to use an artist's or photographer's creative work in printed and online publications of their Mental Health, Addictions, or Spiritual Health Care Strategies. This form has previously been used for the Rising to the Challenge Provincial Mental Health Strategy, the Reclaiming Hope: Manitoba's Youth Suicide Prevention Strategy, the Health and the Human Spirit Spiritual Health Care Strategy.
This form is applicable toญญญญญญญญญญญ any member of the public giving MHSAL permission to use their creative work.
