This form is intended to gather information on Proponent(s) submitting a Proposal to provide Services, to ensure they have read the Request for Proposal, and their acceptance that all statements and representations in the Proposal are true. This form is applicable to Proponents providing a Proposal for Services being requested in the Request for Proposal. This form is processed by Manitoba Health, Seniors and Active Living.
