The purpose of this form is to confirm and document information about the My Health Team as first described in the Expression of Interest to ensure that it is ready to move to the service planning stage. This form is applicable toญญญญญญญญญญญ members of a new My Health Team including individuals from private primary care clinics, Regional Health Authorities, community organizations and service delivery organizations. Manitoba Health, Seniors and Active Living processes this form.
