This policy is applicable to organization's once every five years, providing they are linked to a Manitoba Health, Seniors and Active Living Program. This policy is enforced by Manitoba Health, Seniors and Active Living.

This form is intended to gather information on organization's submitting a request to receive a Hospitality Grant from Manitoba Health, Seniors and Active Living. This form is applicable to organization's once every five years, providing they are linked to a Manitoba Health, Seniors and Active Living Program. This form is processed by Manitoba Health, Seniors and Active Living.
