DHSU Medical Supplies Request and Justification Form (EIA/CLDS/CDS)
This form is completed by regulated health professionals to request medical supplies for a child enrolled with Children's disABILITY Services. The form requires client and health professional information as well as a listing of requested supplies. Completed forms are submitted to the Disability and Health Supports Unit for assessment.
