DHSU Medical Equipment Request and Justification Form (EIA/CLDS/CDS)
This form is completed by regulated health professionals to request medical equipment on behalf of a child enrolled with Children's disABILITY Services. The form requires client and health professional information, as well as a justification letter/report in support of requests for specialized equipment. Completed forms are submitted to the Disability and Health Supports Unit for assessment.
