This document is a sample of the application form, provided at the Parent Information Session only, completed by parents or guardians to apply for Autism Outreach services in Winnipeg for children enrolled with Children's disABILITY Services who have a diagnosis of autism spectrum disorder. The form asks for basic information about a child and for parental consent for the release and sharing of information.
