Community Living disABILITY Services - Intake - Appendix A - Eligibility Letter
This document is the standard letter of notification issued by the Department of Families to inform an individual that they are eligible for the Community Living disABILITY Services program. The document also informs the individual that the program will be contacting them to schedule a Supports Intensity Scale assessment interview.
