This form is completed by regional program managers within the Department of Families when submitting a request to the Admissions Approval and Prioritization of Funding Committee for funded support services for a Community Living disABILITY Services program participant that cost in excess of the prescribed dollar amounts that regional program managers are authorized to spend per participant.
