Personal Plan - Joint Planning Program - Notice and Consent
This form grants the consent of an EIA participant who is receiving Adult Probation Services for the use and disclosure of personal and personal health information, as part of joint planning to develop a Personal Plan. The form must be signed by the participant and Province of Manitoba staff.
