Forms and Policies

Individuals who have previously held Manitoba certification including successfully participating in a recognized ODS training program may become recertified. To initiate your recertification, you must complete and return this application form and remit payment to MOPIA. An annual fee is charged to all certification holders as specified under the Manitoba Ozone Depleting Substances and Other Halocarbons Regulation 103/94.
