This form must be completed by individuals or corporations applying for a provincial park vacation home lease/permit, to make a declaration related to whether or not the individual applicant or corporation shareholder (or their immediate family member) is an employee or officer of Manitoba Sustainable Development or Manitoba Finance, or holds an office under The Crown Lands Act.
