Worker Advisor Office - Authorization Letter
When injured workers ask the Worker Advisor Office to assist them in appealing a decision on their Workers Compensation Board (WCB) claim, the WCB file needs to be reviewed. The worker's written permission is required for the Worker Advisor Office to obtain a copy of a WCB file. This letter includes an authorization form workers can fill out to provide their consent.
