Forms and Policies

The report is printed from the Educational Information System (EIS) Collection, which lists pupils coded as requiring specially equipped vehicles.  School divisions are required to describe the special equipment (e.g. wheelchair, walker, crutches, etc.) related to a pupil's mobility impairment to verify that the pupil requires transportation in a specially equipped vehicle and submit the completed form to the Schools Finance Branch.
