This form must be completed by Researchers that are submitting an application to the Health Information Privacy Committee if they believe that there may be a perceived conflict of interest, such as where the researcher has multifaceted professional roles and obligations. This policy is applicable to researchers looking to access personal health information maintained by the government or a government agency. This policy is enforced by the Health Information Privacy Committee.
