Forms and Policies

This form is for the purpose of an owner of X-Ray equipment to register the equipment with Manitoba Health, Seniors and Active Living.  The owner is to notify the Radiation Protection Division of any new or change in equipment, its use or location prior to such change.  This form is enforced by CancerCare Manitoba.
