This form has been developed to guide Salmonella case investigations. This questionnaire is very comprehensive and is intended to help to identify possible sources of exposure and detect outbreaks in a timely fashion. This form is applicable to health professionals working in Manitoba. This form is enforced by Manitoba Health, Seniors and Active Living.
