This form is required to be completed when there is suspicion of a Vaccine Preventable or Respiratory Infections outbreak. This form is applicable to health professionals working in Manitoba that are not registered users of the Canadian Network for Public Health Intelligence Outbreak Summaries application. This form is enforced by Manitoba Health, Seniors and Active Living.
