Outbreak Summary Report (NOT Enteric or Respiratory/ Vaccine Preventable Disease Outbreaks
This form is required to be completed when there is suspicion of a communicable disease outbreak that is not an enteric, respiratory or vaccine preventable disease outbreak. This form is applicable to health professionals working in Manitoba. This form is enforced by Manitoba Health, Seniors and Active Living.
