This form is used for ordering seasonal influenza and pneumococcal polysaccharide vaccines from the Provincial Distribution Warehouse. This form is applicable to physicians, nurses, pharmacists and other health professionals providing immunization services. This form is processed by Manitoba Health, Seniors and Active Living.
