Record of User Activity (RoUA) Request Form
This form must be completed and submitted to the department by individuals that are seeking a record of user activity showing accesses to their personal health information that is maintained in a departmental health information system. This form is applicable to all individuals seeking a record of user activity from the department. This form is processed by the branch that is responsible for the health information system in question.
