Health Information Privacy Committee Protocol Amendment Form
This form is required when a researcher is making any significant changes to the originally proposed study design for a research project that was approved by the Health Information Privacy Committee. This form is applicable to researchers that have received an approval for their research project. This form is enforced by the Health Information Privacy Committee.
