This form (waiver) provides a written acknowledgement that persons involuntarily committed under The Mental Health Act have been notified of their hearing before the Mental Health Review Board, and are waiving the required 7-day Notice of Hearing before the Mental Health Review Board.
This form is applicable toญญญญญญญญญญญ the person who should have received the Notice of Hearing scheduled before the Mental Health Review Board, under Section 53 of The Mental Health Act.
This form is processed by the Mental Health Review Board of the Department of Health, Seniors and Active Living.
