This form is used by practitioners to submit their banking information in order to facilitate direct deposit of their fee for service income from claims submitted. This form is applicable to all practitioners who submit fee-for-service claims for services rendered to Manitoba Health, Seniors and Active Living. This form is processed by Manitoba Health, Seniors and Active Living.
