Forms and Policies

This form is used to authorize another person to represent the appellant (the person or corporation making the appeal) on the appeal.  This person can be a lawyer or another individual (such as a family member or friend). This form is filled out by members of the public, or corporations. This form is processed by the Manitoba Health Appeal Board.
