Guidelines for Completing a Request for Access to Personal Health Information Held by the Government of Manitoba
This policy is to be used in conjunction with the application "Request for Access to Personal Health Information Held by the Government of Manitoba" and provides guidelines and requirements for completing the form. This policy is applicable to researchers submitting the above named application to the Health Information Privacy Committee. This policy is enforced by the Health Information Privacy Committee.
