This policy is a template for an agreement that outlines the rationale for, and details and conditions of, the disclosure of personal health information held by Manitoba Health, Seniors and Active Living (MHSAL) to another Organization. This policy may be applicable to organizations that the government is authorized to disclose personal health information to under section 22(2) of the Personal Health Information Act. This policy is enforced by Manitoba Health, Seniors and Active Living.
