This policy is a template for an agreement required under section 25 of The Personal Health Information Act when a Trustee requires an Information Manager for the purpose of processing, storing or destroying personal health information or providing the trustee with information management or information technology services. This policy is applicable to organizations or individuals that are acting as an Information Manager for Manitoba Health, Seniors and Active Living. This policy is enforced by Manitoba Health, Seniors and Active Living.
