Forms and Policies

This policy constitutes an agreement of the conditions under which personal health information from Manitoba Health, Seniors and Active Living will be disclosed to a researcher in accordance with the provisions stated in The Personal Health Information Act.     This policy is applicable to researchers that have received approval from the Health Information Privacy Committee to access personal health information. This policy is enforced by Manitoba Health, Seniors and Active Living.
