Appendix A to Residential Form of Offer to Purchase - Property Disclosure Statement
This form is an appendix to the residential offer to purchase prescribed under The Real Estate Brokers Act. The form is used by the seller of a single family residential house to disclose information on the condition of the property, to the best of the seller's knowledge.
