First Nations schools use this form to confirm that they are meeting requirements set by the department and Indigenous and Northern Affairs Canada in order to receive (and maintain) a Manitoba provincial school code enabling First Nations schools to submit high school marks to Manitoba Education and Training. The form is not available in French.
