Notice to Birth Father
This document informs a man that he has been named as the birth father of a child and advises him of plans for the child to be adopted. The form describes how to be legally named the father of the child, whom to contact to obtain information about plans for the child, including pre-adoption birth registration information that may be available to the birth father and adopted child at age 18.
