DHSU Wheelchair Seating Component Request and Justification Form (EIA/CLDS/CDS)
This form is used to request wheelchair seating components for participants of Employment and Income Assistance, Community Living disABILITY Services and Children's disABILITY Services. Requests for wheelchair seating components must be completed by an occupational therapist or physiotherapist. Requests must be submitted to the Disability and Health Support Unit for assessment.
