Residential Care Licensing - Residential Care Facility Renewal/Re-Licence Application
This form is completed by residential care facility operators two months prior to the expiry of a Licence or Letter of Approval to, in the case of a Licence, renew the Licence, or in the case of a Letter of Approval, renew the Letter of Approval.
