Rewarding Volunteer's Benefit (RVB) Volunteer Log Sheet
This document is a Volunteer Log Sheet recording activities, time spent volunteering, and confirming volunteer participation of an EIA participant with a disability. This form must be completed by the volunteer agency/placement site and sent to EIA staff to ensure proper administration of the Rewarding Volunteer Benefit.
